To receive your Mattel Toy rebate check, send
proof of purchase (UPC code) and dated sales
receipt with Mattel item circled to: JCPenney
Rebate, P.O. Box 8715, St. Cloud, MN 56398-
8715. Toys must be purchased before 11/15/96.
Your rebate request must be postmarked by
December 31, 1996. Please allow 8-10 weeks for
receipt. Offer void where prohibited, taxed or
otherwise restricted. Offer good only in the USA.
Rebate amount not to exceed $110. Limit one
